FERTILYSIS COUPLES MICROBIOME: iT TAKES TWO TO TANGO

For a comprehensive evaluation of both partners' infections, we offer the FERTILYSIS Couples Microbiome test. This test detects active or chronic sub-clinical infections of the endometrium in women, which can impair implantation and lead to recurrent miscarriages, infertility, and pregnancy complications. It also detects infections in the upper genital tract, often missed by conventional testing. The test analyzes menstrual blood for over 25 bacterial, fungal, and pathogenic species without requiring a biopsy. Similarly, the male partner's semen sample is tested for over 35 species and groups of pathogens, as infections can impact sperm quality and fertility. This at-home testing method ensures accurate diagnosis and tailored treatment plans for both partners.

The FERTILYSIS Couples microbiome test combines two very important panels:

The FERTILYSIS Female Microbiome panel

The FERTILYSIS Male Microbiome panel

The Role of a Healthy Baby-friendly Microbiome

A healthy pregnancy relies on a balanced uterine environment dominated by beneficial Lactobacilli bacteria. These bacteria protect the uterus, regulate pH, and promote antimicrobial peptides. They also support immune system regulation, crucial for a pregnancy-friendly environment.

Chronic Endometritis (CE) is often symptomless but can lead to inflammation, making successful implantation difficult and increasing the risk of miscarriage. It can also harm the embryo directly or lead to serious complications later in pregnancy.

An unhealthy uterine microbiome can trigger inflammation, shifting from a pregnancy-friendly state to a hostile one. This dysregulation can promote immune responses detrimental to pregnancy, increasing the risk of miscarriage and autoimmune disorders.

The role of the male microbiome

Bacterial infections from male partners can jeopardize female partners' microbiome and reproductive health. Seminal microbiota not only regulate men's reproductive health but also impact couples and offspring health via microorganism transfer. Semen serves as a medium for bacteria and virus transmission, leading to sexually transmitted diseases. Various microorganisms interfere with reproductive functions in both sexes. Prostatitis, ranking third among male infertility causes, leads to inflammation in the male genital tract, affecting spermatogenesis. This inflammation triggers high-grade oxidative stress, altering sperm metabolism, motility, and DNA integrity. Prostatitis-induced local oxidative stress correlates with systemic oxidative stress, associated with cancer and cardiovascular diseases. Early-onset prostatitis significantly increases the risk of BPH and prostate cancer.

When experiencing symptoms such as:

  • Abnormal vaginal discharge
  • Unusual vaginal odor
  • Itching or irritation in the genital area
  • Pain or discomfort during urination or sexual intercourse
  • Abnormal bleeding between periods

When you have been diagnosed with:

  • Blocked tubes of hydrosalpinx
  • Positive CD138 biopsy test
  • Confirmed or suspected Endometritis
  • Confirmed or suspected Endometriosis
  • Recurrent Vaginitis, Bacterial Vaginosis (BV) or UTIs
  • PID

When you have abnormal sperm (bad or borderline semen analysis results)

  • Low count
  • Poor motility
  • Low Morphology

When you have a high DFI (DNA Fragmentation Index)

  • High levels of sperm DNA damage linked to infection-linked inflammation and oxidative stress, are linked to reduced chances of natural conception, and poorer outcomes in assisted reproductive techniques like IVF

When you have a history of Prostatitis

  • Prostatitis is often linked to infection, as it involves inflammation of the prostate gland, usually due to bacterial infection. Bacteria can enter the prostate through urinary reflux, urethral stricture, or direct extension from nearby structures and can trigger inflammation and the symptoms associated with prostatitis

Probiotic & Prebiotic supplementation:

  • Following or in parallel to antibiotic treatments, it has been shown that male normal flora supplementation can be very beneficial in ameliorating abnormal sperm parameters because of a disrupted microbiome. For example it has been shown that a 6-week course with Lactobacillus and Bifidobacterium improved sperm motility and reduced the percentage of sperm DNA fragmentation in asthenozoospermic males while daily administration of Lactobacillus paracasei, arabinogalactan, fructo-oligosaccharides, and l-glutamine over a period of 6 months ameliorated sperm count and motility, and reduced the rate of atypical forms, in comparison to the placebo-receiving group.

The Fertilysis Microbiome test aims to thoroughly analyze the uterine and vaginal microbiomes, identify any imbalances or dysbiosis, and intervene therapeutically. This involves eliminating pathogenic infections, restoring optimal levels of Lactobacilli, and promoting a healthy microbiota to ensure a favorable environment for pregnancy.

Once we have your results, we will take into consideration the combined data from both your uterine and vaginal microbiomes and assess their clinical significance in order to provide you with a diagnosis and its clinical significance level.

If you have a Fertility Specialist:

  • If therapeutical intervention is recommended, on your comprehensive report you will find personalized suggestions for antibiotics, probiotics, antivirals, antifungals, and more, which you can then discuss with your doctor or IVF or fertility specialist.
  • Following the completion of treatment, retesting may be recommended to assess the efficacy of the interventions and ensure the restoration of a balanced and healthy microbiome. This comprehensive approach, under the guidance of your healthcare provider, aims to optimize your reproductive health and fertility outcomes.
  • Keep in mind that if requested, we can work with your fertility Specialist to provide assistance with test result interpretation and treatments. Your doctor may feel free to contact us for any required additional information.

If you don’t have a Fertility Specialist:

  • For those seeking additional interpretation, clinical advice, or direct prescription services, we offer a follow-up online consultation for a small fee, to discuss your results together, prescribe treatments and if needed, plan your next steps and strategy together.
  • If you prefer a more in-person approach and you like travel, we will be very happy to have you visit us in our clinic in Athens for an appointment, more testing and specialized treatments.

Is your infertility really ‘’unexplained’’ or are you still undiagnosed? If you have been struggling with infertility and haven’t been getting convincing answers, chances are you are still missing a piece of the puzzle.

Did you receive treatment? Retest to confirm.

Retesting is essential for evaluating the effectiveness of your treatment and making necessary adjustments. It allows us to determine if the treatment was effective or needs to be repeated, preventing recurring infections. Through careful reevaluation, we ensure that your personalized treatment plan remains effective. Additionally, it helps to identify any microbiome imbalances that may have arisen post-treatment. Consequently, it is recommended that the retesting occurs no later than 1 – 2 months following completion of treatment, and, in any case, it should take place no longer than 6 months from the initial test.

Is FERTILYSIS Male Microbiome testing the same as semen culture?
No, FERTILYSIS Male Microbiome testing, and semen culture are not the same. FERTILYSIS Male Microbiome testing examines the microbial composition of the male reproductive system, to assess the presence of bacteria, viruses, fungi, and other microorganisms. This testing provides insights into the microbiome's impact on fertility and reproductive health. In contrast, semen cultures are used to identify suspected bacterial infections, with significant constraints on being able to culture only certain microorganisms. semen cultures cannot be used during antibiotic treatment, while the sample needs immediate analysis due to the requirement of live microorganisms. Most importantly, semen cultures do not provide any data on the microbiome's composition which makes this test more useful as a diagnostic test for infections and disease rather than investigation of infertility.
Ιs it possible to do this test if I live in the USA?
