Genetics & Infertility: Connecting the Dots

Sperm FISH (fluorescent in situ hybridization) analysis: Precision medicine testing for the evaluation of male fertility.

FISH analysis of Chromosomes 13, 18, 21, X and Y for sperm aneuploidies allows chromosomal analysis of sperm cells to determine if there is any increasing risk of chromosome abnormalities which has been associated with reduced fertility, recurrent miscarriages, or an increased risk of passing genetic disorders to offspring. Sperm FISH analysis is typically recommended for couples experiencing recurrent pregnancy loss, repeated IVF failure, or male infertility issues such as severe oligozoospermia (low sperm count) or teratozoospermia (abnormal sperm morphology). Additionally, individuals with a history of genetic disorders or known chromosomal abnormalities may also benefit from this testing to assess the risk of passing on genetic abnormalities to offspring.

This test is particularly useful in cases of male infertility or recurrent pregnancy loss, where chromosomal abnormalities in sperm may contribute to reproductive problems. Chromosomes 13, 18, and 21 are assessed because abnormalities in these chromosomes are associated with conditions like Down syndrome (Trisomy 21) and other chromosomal disorders. Chromosomes X and Y are analyzed to detect sex chromosome abnormalities, which can affect fertility and embryo development. Especially in cases of significant oligozoospermia (low sperm count) where assisted reproductive techniques like in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) are used, FISH analysis provides valuable information about the genetic integrity of sperm, helping fertility specialists understand the potential risks of using it for fertilization of egg. By identifying aneuploid sperm (sperm with abnormal chromosome numbers), healthcare providers can make informed decisions about the most appropriate fertility treatments for couples facing infertility or pregnancy loss related to sperm chromosomal abnormalities.

Consider Sperm FISH chromosomal analysis:

If you suffer from:

  • Low sperm count (oligozoospermia)
  • Low mobility (astenozoospermia)
  • Or low morphology (teratozoospermia).

Especially if ICSI is used in combination with the above! Men with severe spermatogenesis impairment can successfully father children using intracytoplasmic sperm injection (ICSI). Oligozoospermic patients have higher sperm aneuploidy rates, increasing the risk of transmitting chromosomal abnormalities via abnormal sperm injection.

Additionally, sperm FISH analysis is recommended for couples with a history of:

  • Recurrent pregnancy loss
  • Repeated IVF failure

Risk factors for sperm aneuploidies involving chromosomes 13, 18, 21, X, and Y include:

  • Advanced paternal age
  • Environmental exposures such as radiation or certain chemicals,
  • Lifestyle factors like smoking and excessive alcohol consumption
  • Certain medications
  • And underlying genetic conditions such as Klinefelter syndrome or structural abnormalities in the sex chromosomes

In cases where there are findings, genetic counseling and testing are recommended to assess the risk of passing the translocation to offspring. Options for managing fertility and reducing the risk of recurrent pregnancy loss may include preimplantation genetic testing (PGT) during in vitro fertilization (IVF) to select embryos with balanced chromosome numbers before transfer to the uterus.
