FERTILYSIS Recurrent Pregnancy Loss (RPL): All-Inclusive Approach
In order to prevent recurrent pregnancy loss (RPL), including implantation failure, chemical pregnancies and miscarriages, we need to make sure that the uterine environment is optimal for pregnancy in two ways: Firstly, the uterine microbiome needs to be perfectly balanced, dominated by normal flora lactobacilli and free of any pathogenic or potentially pathogenic bacteria and microorganisms. Secondly, your immune system must be properly adapted for the accommodation of pregnancy without exhibiting immunological hostility towards the embryo but also to actively facilitate the implantation and placentation process.
The FERTILYSIS RPL (recurrent pregnancy loss) PANEL is our most comprehensive one yet, combining our two largest and most important unexplained infertility investigation panels, the FERTILYSIS Reproductive Immunology Full and the FERTILYSIS Female Microbiome: Full, at a premium price.

WE GOT YOU.
Through the array of tests included in the FERTILYSIS RPL PANEL, we are able to test whether you have any active or chronic sub-clinical infections of the endometrium (endometritis) or any serious imbalance of your microbiome both of which may impair implantation and could lead to recurrent miscarriages, infertility and serious pregnancy complications. Furthermore, this panel offers the most comprehensive Reproductive Immunology testing which allows detection of both idiopathic reasons (Autoimmunity Dysfunction) or risk factors due to “incompatibility between embryo genetics and your immune system (Alloimmunity Dysfunction) associated with reproductive immunopathology, such as recurrent spontaneous abortions, idiopathic infertility, and implantation failures in both natural and assisted reproduction attempts.
The FERTILYSIS RPL panel combines the two largest and most important unexlained investigation panels:

The Role of a Healthy Baby-friendly Microbiome
A healthy pregnancy relies on a balanced uterine environment dominated by beneficial Lactobacilli bacteria. These bacteria protect the uterus, regulate pH, and promote antimicrobial peptides. They also support immune system regulation, crucial for a pregnancy-friendly environment.
Chronic Endometritis (CE) is often symptomless but can lead to inflammation, making successful implantation difficult and increasing the risk of miscarriage. It can also harm the embryo directly or lead to serious complications later in pregnancy.
An unhealthy uterine microbiome can trigger inflammation, shifting from a pregnancy-friendly state to a hostile one. This dysregulation can promote immune responses detrimental to pregnancy, increasing the risk of miscarriage and autoimmune disorders.
The role of your Immune system
A potentially hazardous immunological response during pregnancy is more probable when the mother exhibits elevated levels of pbNK cells, facilitating an immunological attack against the fetus. Heightened pbNK cytotoxicity, combined with a propensity for a Th1 proinflammatory response over the favorable Th2 anti-inflammatory counterpart, poses risks for pregnancy. Elevated concentrations and increased activity of cytotoxic pbNK cells, alongside a dominance of the pro-inflammatory Th1 response, have been linked to reproductive immunopathology, including conditions like recurrent spontaneous abortions (RSA), idiopathic infertility, and implantation failure in both natural and assisted reproduction attempts. This panel also includes the HLA-C-KIR mismatch test which allows comparison between the mother’s KIR genotype and paternal HLA-C. A potential mismatch here, can result in very serious repercussions including infertility and recurrent miscarriage, increased risk of pregnancy complications, intrauterine growth restriction, preterm birth or even unexplained stillbirth. All the potentially detectable problems that this «super-panel» can reveal, each have their own specialized treatments.
A couple may consider our FERTILYSIS RPL testing if they experience recurrent pregnancy losses, unexplained infertility despite regular attempts to conceive, or implantation failures in assisted reproductive technology (ART) cycles such as in vitro fertilization (IVF). Additionally, if either partner has a history of autoimmune disorders or immunological conditions, or if there are indications of immune system dysregulation, such as chronic inflammation or suspected immune-related reproductive issues, testing for reproductive immunology problems may be warranted.

Based on your results, we can provide personalized recommendations and immunomodulatory treatments to optimize the chances of a successful pregnancy. By understanding the role of the immune system in maternal-fetal tolerance, interventions can be tailored to address specific immunological factors that may impact pregnancy outcomes. We will also take into consideration the combined data from both your uterine and vaginal microbiomes and assess their clinical significance in order to provide you with a diagnosis and its clinical significance level.

Main Treatments:
Treatment for reproductive immunology problems typically require a healthcare professional specialized or well-versed in reproductive immunology and its specialized therapies. Each detected problem has its own recommended treatment(s) and each treatment is tailored to address specific immune system dysfunctions and optimize fertility outcomes. If you don’t have access to specialized treatment protocols then we will be able to provide protocols for the treatments we consider appropriate based on your Fertilysis results and your medical history following a specialized follow-up, on-line consultation with our specialist. However, even so, you will need to have a local healthcare professional who will be able to help implement and supervised said therapies included in our protocols and prescriptions.

Is your infertility really ‘’unexplained’’ or are you still undiagnosed? If you have been struggling with infertility and haven’t been getting convincing answers, chances are you are still missing a piece of the puzzle.
Did you receive treatment? Retest to confirm.
Retesting is essential for evaluating the effectiveness of your treatment and making necessary adjustments. It allows us to determine if the treatment was effective or needs to be repeated, preventing recurring infections. Through careful reevaluation, we ensure that your personalized treatment plan remains effective. Additionally, it helps to identify any microbiome imbalances that may have arisen post-treatment. Consequently, it is recommended that the retesting occurs no later than 1 – 2 months following completion of treatment, and, in any case, it should take place no longer than 6 months from the initial test.
