Persistent Endometrial Inflammation: A Case Study of Successful Treatment with Uterine Antibiotic Washes
Today’s case highlights the importance of advanced diagnostics and targeted treatment in overcoming chronic endometrial inflammation (CE). The patient experienced several failures over two years. Despite undergoing various standard fertility treatments and numerous tests, she was ultimately given a diagnosis of ''unexplained infertility''.
The Challenge: Unexplained Infertility & Repeated Failures
B*, a 36-year-old woman, had been trying to conceive for over two years—both naturally and through IVF. She experienced multiple chemical pregnancies and recurrent implantation failure despite undergoing several timed cycles and two IUIs.
Basic fertility workups yielded no clear answers. Even her CD138 test, commonly used to detect chronic endometritis, was negative. She was left with an unsettling ‘’diagnosis’’: unexplained infertility.
Microbiome Testing: Finding the Root Cause
Determined to dig deeper, she pursued comprehensive microbiome testing with Fertilysis. The results:
- Chronic Endometritis (CE) was present—despite the negative CD138 test.
- Infections were detected in both the vaginal and uterine microbiome.
- Enterobacteria and Staph infections were found in both the vagina and uterus.
- Gardnerella infections were detected exclusively in the uterus.
The inflammation in her endometrial lining was likely preventing successful embryo implantation—a factor often missed in standard fertility workups.
The Treatment: Targeted Antibiotic Therapy & Uterine Washes
B* was prescribed oral antibiotics and probiotics to help restore a healthy vaginal and gut microbiome. She was also advised to use vaginal antifungal creams to prevent antibiotic-induced thrush.
But when she re-tested, the infection persisted.
With a frozen embryo transfer (FET) scheduled within two months, we needed a stronger, more direct approach.
The solution? A diagnostic hysteroscopy confirmed the presence of chronic endometritis, leading to a course of 6 uterine antibiotic washes—a more targeted and effective treatment to eliminate the persistent inflammation.
The Results: A Healthy Endometrial Microbiome & Pregnancy
After completing the 6 uterine washes, follow-up testing showed:
- No inflammation
- A Lactobacillus-dominant microbiome (95%)
- A receptive uterine lining ready for implantation
The result? A successful frozen embryo transfer (FET)!
B* is now 20 weeks pregnant—after years of unexplained fertility struggles.
Why This Matters for YOU
- Chronic endometrial inflammation is often overlooked. If you’ve had multiple failed cycles or chemical pregnancies with no clear cause, this could be the missing link.
- Oral antibiotics aren’t always enough. In persistent cases, targeted uterine washes can significantly improve success rates.
- Specialized testing is a game-changer. If standard tests have left you without answers, a microbiome test should be your first go-to test!
Could this be the solution for you?
If you're struggling with unexplained infertility, recurrent implantation failure, or failed IVF cycles, it’s time to explore specialized fertility diagnostics and treatment options.
